Guidelines for Co-op Faculty Advisors
As the Co-op Faculty Advisor for your department, students who have been hired
for a Co-op position are required to notify you.
Each student will bring a Student Agreement Form from Career Services for
you to sign and approve the Co-op assignment. You will need to assess the potential of the learning
experience, determine its relevance to the major, and approve the student for
Co-op participation. You will give
each student all required registration information including course number,
number of units, call number and course requirements.
Many Co-op Faculty Advisors use the Co-op
course requirements provided online. This includes the Co-op Calendar, Work
Information Form, Progress Report, Co-op Professional Report, Student
Evaluation, and Supervisor Evaluation.

Calendar
The Co-op Calendar provides students with important dates they need to be
aware of while on their Co-op assignment (registration, deadlines, etc.) -
Transition to the Workplace
This information may help students in making an effective
transition from school to the workplace.
It includes tips on relocation, getting started, being a good employee,
maximizing the Co-op experience, and other student service information.

Work
Information Form
The Work Information Form is used by students to send you their address
while on Co-op and their initial impressions of the job. You may use the information on the
form to mail future registration information.

Progress Reports
Progress Reports assist students in evaluating their overall Co-op
experience. Students are
required to complete these monthly reports to inform you of their on-going
progress and any problems that require assistance.
If there are any immediate problems or concerns, the student should
contact you directly. Due:

Professional Report
The
Professional Report gives students experience in writing both professional
and general reports and encourages them to reflect on the educational
benefits of their experience.
As the Co-op Faculty Advisor, you may allow the student to follow the
company’s normal technical report format and review procedure instead of the
standard Co-op Professional Report Guidelines.
Students are required to submit two hard copies, one copy for you and one to
be forwarded by you to Career Services.

Student Evaluation
The
Student Evaluation is used to assess how much the Co-op experience affected
the clarity of the students’ career plans and goals, and their present level
of skill in specific learning outcome areas.
In addition, the students’ feedback assists with program quality and

Supervisor Evaluation
The Supervisor Evaluation is intended to be a realistic assessment of
the students’ present level of skill in specific learning outcome areas.
It is to be completed by their immediate Co-op supervisor and returned to
you, the Co-op Faculty Advisor.

Debriefing
During their first week back at school, students should schedule
a "Debriefing Session" with you.
This is usually a one-hour meeting to discuss the Co-op experience and may
include other returning Co-op students in your department as well.

Submission Requirements
Students will
submit an original and a copy of their Co-op Professional Report, Student
Evaluation and Supervisor’s Evaluation to you during the first week back to
school.
